Practice Nurse

The Heron Practice 

Salary £35,000 to £45,000 a year depending upon experience 

Hours: 37½ hours/week – Monday to Friday (part-time will be considered)

Please note: Start/finish times will be agreed with the successful candidate however a 7.30pm finish to cover extended hours will be required midweek, days to be discussed

The Heron Practice, is a multi-award-winning teaching Partnership, serving patients in the community for more than 30 years, in the Woodberry Wetlands region looking after circa 12,000 patients.

Located close to Manor House underground station, offering a friendly and supportive working environment.   Our full complement of staff include Salaried Doctors, Clinical Pharmacist, Practice Nurses, Health Care Assistants, Patient Services staff round out our team.

Job summary 

We are looking for a highly motivated and enthusiastic Practice Nurse to join our existing nursing team in providing excellent service and support to our patients.

Applicants will need to demonstrate they have skills and experience in the following areas:

  • Chronic disease management
  • Family Planning/Sexual Health
  • Vaccinations and immunisations for adults and children
  • Cervical cytology screening
  • Travel vaccinations
  • Ear syringing
  • Wound care
  • Health promotion and screening

Excellent interpersonal skills and IT literacy are essential, as is the ability to work flexibly as part of a dedicated team and to manage your own workload. The practice uses the EMIS Web clinical system.

Job Summary

The post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of preventative nursing activity to the practice population. As autonomous practitioners the nurse is responsible for the care they deliver, demonstrating critical thinking and skills in clinical decision making in the management of patients. They will work collaboratively within the practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing nurse leadership as required.

Main duties of the job: 

You will work within your scope of clinical practice and :

  • Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being
  • Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate
  • Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs
  • Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols
  • Work with patients in order to support adherence to prescribed treatments
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care
  • Support and manage health needs of women presenting for family planning and cervical cytology consultations
  • Recognise, assess and refer patients presenting with mental health needs in accordance with the national guidelines
  • Implement and participate in vaccination and immunisation programmes for both adults and children
  • Advise, support and administer vaccinations where appropriate for patients travelling abroad
  • Promote and deliver evidence-based care for patients presenting with aural conditions
  • Meet the needs of patients presenting for opportunistic wound care

Person Specification 

Qualifications 

Essential 

  • Qualified and registered Nurse
  • Relevant nursing/ health Degree in Primary Care
  • Registered with the Nursing and Midwifery Council

Experience 

Essential 

  • Minimum of 1 years post registration experience
  • At least 1 years recent experience of working in Primary Care
  • Experience of nurse led management of long term clinical conditions
  • Experience of implementing protocols and clinical guidelines

Desirable 

  • Minimum of 2 years experience of working in a primary care environment
  • Experience in triage
  • Experience in paediatric care
