The Heron Practice use Coordinate My Care to coordinate urgent care for vulnerable patients. You may be offered a longer appointment to discuss your care in more detail. This is called a "Time to Talk" appointment.

You can start your own Coordinate My Care record. It starts with you filling in an on line Advance Care Planning questionnaire called myCMC.  myCMC then goes to a doctor or nurse who knows you who can complete the Coordinate My Care (CMC) plan by adding relevant medical information and  recommendations for the urgent care services to follow in an emergency. Once completed the plan is approved and is immediately visible to all the urgent care services including 111, out of hours GPs, the ambulance (in their vehicles) and the emergency departments. This way everyone is in the loop with the patient in the middle.
