At present it is an additional offer to the paper red book but in time it will replace it for those who want a digital record. Patient Forum
We are always looking for more patients to join our Patient Forum Group. For further information please contact Mandy Lawrence, Practice Manager, or click on "Further Information Patient Group" on the home page. 

Named Accountable GP
The NHS standard contract for 2015/16 requires that all patients, including children, are allocated a named, accountable GP. The named, accountable GP will have overall responsibility for the care and support that the practice provides for you. This does not prevent you from seeing any GP in the practice as you currently do.
If you would like to know who your named GP is please contact reception.

Change of Telephone Number
It is very important that you let us know if you change your telephone number.
You can do this by completing:
* the on line change of details form at the foot of this page
* the change of details form available from reception

Alternatively you can include the details on your repeat prescription request.

We currently have Drs Clare Baptiste and Elizabeth O'Donovan undertaking their "in service" placement with us. 

Making a Complaint

Complaints

We make every effort to give the best service possible to everyone who attends our practice.

However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible. Simply contact the Practice Manager and she will set all the necessary wheels in motion.

We are continually striving to improve our service. Any helpful suggestions would be much appreciated and a suggestion box is located in the waiting area.
