Intrauterine contraceptive device (COIL) fitting and removals
Unfortunately the practice is no longer providing this service. We apologise for any inconvenience this will cause. Please contact reception if you require any further information or details of other service providers.

16 week Antenatal checks
From 1 April 2019 the practice will no longer provide 16 week antenatal checks, these will be delivered by Homerton maternity service. To arrange your antenatal check please contact your named midwife, if you are unable to contact your named midwife please contact the maternity helpline at the Homerton on 0208 510 5955. We apologise for any inconvenience this will cause.

Named Accountable GP
The NHS standard contract for 2015/16 requires that all patients, including children, are allocated a named, accountable GP. The named, accountable GP will have overall responsibility for the care and support that the practice provides for you. This does not prevent you from seeing any GP in the practice as you currently do.
If you would like to know who your named GP is please contact reception.

Change of Telephone Number
It is very important that you let us know if you change your telephone number.
You can do this by completing:
* the on line change of details form at the foot of this page
* the change of details form available from reception

Alternatively you can include the details on your repeat prescription request.

GP Registrars
As you may be aware the Heron Practice is a teaching practice.
We currently have Dr Clare Baptiste undertaking her "in service" placement with us. 

We are happy to register new patients who have moved into our practice area. We do not discrimate on the basis of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical conditions.

Our practice boundary is broadly covered as follows:

  • NORTH: Seymour Road N8
  • EAST: Stoke Newington Road N16, Stoke Newington High Street N16, Stamford Hill N16
  • SOUTH: Matthias Road N16, Pellerin Road N16
  • WEST:Blackstock Road (up to no.156) N4, Seven Sisters Road (nos.269-523) N4, Wightman Road (up to no.149) N4 

To check whether a specific address is within our catchment area or for further information about our registration procedure please contact our reception team.

Please note if you wish to come into the practice in person to register you may do so between 8.00am and 1.00pm or 2.00pm and 6.30pm. 

You will need to bring proof of address with you in order to register. This can be any one of the following documents and must be dated within the last 4 weeks:

- Bank statement

- Tenancy agreement

- Gas or electricity bill

- Telephone bill (not a mobile)

- Council Tax bill

- Photo ID Driving Licence

When your application has been processed we will ask you to come for a New Patient Health Check appointment. This is so we can record information about your medical history and health requirements. Please bring any information about medicines and children's vaccinations to this appointment.

Pre-Registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct and to provide proof of address.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice with your proof of address.
