Patient Forum
We are always looking for more patients to join our Patient Forum Group. For further information please contact Mandy Lawrence, Practice Manager, or click on "Further Information Patient Group" on the home page. 

Named Accountable GP
The NHS standard contract for 2015/16 requires that all patients, including children, are allocated a named, accountable GP. The named, accountable GP will have overall responsibility for the care and support that the practice provides for you. This does not prevent you from seeing any GP in the practice as you currently do.
If you would like to know who your named GP is please contact reception.

Change of Telephone Number
It is very important that you let us know if you change your telephone number.
You can do this by completing:
* the on line change of details form at the foot of this page
* the change of details form available from reception

Alternatively you can include the details on your repeat prescription request.

GP Registrars
As you may be aware the Heron Practice is a teaching practice.
We currently have Drs Aliya Mustapha and Amy Adesara undertaking their "in service" placements with us.

We use our website to provide patients with an up to date resource for all information relating to our practice. We keep it current with any news, developments & details that are relevant to the practice & our patients.

 As well as a source of information about our practice, the staff & the services we provide, we use the website to provide links to other reliable websites for health related information. If there is topical health information we will also add this to the website.

Please do send us your feedback. We can use your thoughts to improve our online services & further develop the website to make it a more useful, practical application for our patients.       

FLU SEASON 2018/19

This year there are 3 types of flu vaccine for each of the following groups:

  1. Anyone aged 65 and over.
  2. Eligible adults aged 18- 64 years & eligible children 6 months and above who are at increased risk from flu.
  3. Eligible children & young people aged 2 to 17 who are at increased risk from flu.

3 separate flu clinics will be arranged specifically for each group of patients, we are currently inviting patients aged 18- 64 over to attend for their flu vaccination.

AGED 18-64 FLU CLINICS

Our clinics are walk-in, no appointment necessary:

 

Monday 22nd October   8:30am-9:30am

Wednesday 24th October   8:30am-9:30am

Thursday 25th October   8:30am-9:30am

  

We will send invites for the remaining eligible adults aged 18-64 years and children 2-3 years in due course. There will a further over 65 clinic held in November, dates TBC.
