Patient Forum

Named Accountable GP
The NHS standard contract for 2015/16 requires that all patients, including children, are allocated a named, accountable GP. The named, accountable GP will have overall responsibility for the care and support that the practice provides for you. This does not prevent you from seeing any GP in the practice as you currently do.
If you would like to know who your named GP is please contact reception.

Change of Telephone Number
It is very important that you let us know if you change your telephone number.
You can do this by completing:
* the on line change of details form at the foot of this page
* the change of details form available from reception

Alternatively you can include the details on your repeat prescription request.

GP Registrars
As you may be aware the Heron Practice is a teaching practice.
We currently have Drs Suki Shandrakopal, Jane Miles and Aliya Mustapha undertaking their "in service" placement with us.

As well as a source of information about our practice, the staff & the services we provide, we use the website to provide links to other reliable websites for health related information. If there is topical health information we will also add this to the website.

Please find below our “walk-in, no appointment needed” flu vaccination clinic dates and times for 2017.  Please note these are for eligible ADULTS (over 18) only who will be invited in by text and/or letter. If you are unsure whether you are eligible for a flu vaccination please check at reception.


The clinics for children will be held at a later date.
